CONSUMER DIRECTED CARE – LOW CARE PACKAGES
The Australian Government is funding consumer directed care in selected Australian Government community aged care programs. Under this initiative, the government will fund Churches of Christ Care to deliver innovative service models, which provide care recipients and their carers with greater control over the design and delivery of the care and services they receive.
Consumer (or self) directed care (CDC) allows people to have greater control over their own lives by allowing them, to the extent that they are capable and wish to do so, to make choices about the types of care services they access and the delivery of those services, including who will deliver the services and when.
The services provided as part of a CDC low care are designed to meet an older person’s needs such as:
- shower assistance
- meal preparation
- laundry
- home help
- gardening
- medication support
- transport to medical appointments
- wellbeing and lifestyle support.
Your role as a care recipient will be to identify the services that you need and the budget for the year. You will be given support to do this by a coordinator from a service which you choose to deliver your services. The coordinator will assist you to plan the services that best meet your needs.
